Pre-consultation intake form
So that we may prepare for your visit better and to ensure that you get the best out of the consultation, please note that before your first visit, it is highly encouraged to fill our intake assessment. We strongly encourage you to do this before your visit.
When completing the form, please answer the questions carefully, thoughtfully, and accurately. The information you provide will help us to begin to understand your current state of health and determine which treatments or therapies are best suited for you.
Once you fill out the form, a patient care coordinator will be in touch with you. They will share more about how we work with patients discuss pricing with you and will match you up with a practitioner that suits your needs.

Important information
for all appointments
Before your appointment, please consider the following information

Obtain a copy of all your medical records and/or test results from previously seen doctors, laboratories or health centers and carry them along with you.

Come with all your prescriptions for medications, herbal medicines or supplements that you are currently taking or come along with the physical medications or treatments.

Preferably come after an 8 – 10 hour fast (8 to 10 hours) from your last meal. This is because some tests ordered may require a fasting state like a lipid profile or fasting blood sugar. Lab tests that require fasting usually will require an extra visit if the patient is not prepared.

Please keep time. It is advisable to arrive at least 15 minutes before your scheduled appointment time to allow room for registration and any measurements that have to be taken

Please also note, that we are very strict on time. Any late arrivals may result in a shorter service or no service at all. This is important to ensure all clients are served well.
FAQs about our consultations
1. How do I become a patient?
To get started on the healing journey, we ask you to click this schedule an appointment button or call our clinic at +25670145450 or +256761000450 to find out more about how we can address your health concerns. A patient care coordinator will assist you to make sure that the care we provide fits your health needs.
2. Do you accept insurance?
We currently do not accept insurance. Some insurance providers may refund medical services and laboratory tests performed by our clinicians. We can however provide a detailed receipt for services performed for you to submit to your insurance provider for possible reimbursement.
3. Can the doctors at The Wellness Clinic be my primary care doctor?
We require that you have your own primary care doctor/physician. While our doctors are qualified medical doctors and can provide medical care, they do not provide acute care services. We will work with you closely as consultants and coaches in preventive, nutritional and functional medicine to help you address the roots of chronic health problems. Our doctors can and often will work hand in hand with your primary care doctor if required.
4. What is the charge for a consultation at your clinic?
Our consultation fees depend on the care provider you would like to see, the type of consultation and the duration of your consultation. The duration of our consultation appointments ranges from 20 minutes to 120 minutes.

20 minutes – UGX 100,000
40 minutes – UGX 200,000
60 minutes – UGX 300,000
Our Functional medicine consultation fees for doctors are as follows
45 minutes – USD 100

7. Do you work with cancer patients?
Our doctors and team work with patients to help create an environment in their body where cancer is less likely to grow. For those who are on cancer treatment we can work to support the oncologist team and focus on nutrition, neutraceutical and lifestyle modification to strengthen the immune system, enhance detoxification and support the patient through their treatment. Post treatment, we can work to decrease the risk of recurrence. However, we don’t have specific treatment plans for cancer.
8. Do you work with children?
Yes. Our team work with children to support their health and wellness needs. Our focus is mostly on nutritional and lifestyle interventions to support a healthy weight and to help with other health problems such as autism, ADHD and other health challenges. We will always require that your child has the support of a qualified pediatrician where necessary.
